Dear Sir,
I, Mr. Alim have been working for this bank for the last three years. In this period of time there has not been any kind of irregularity to the job from me. I sincerely hope I have satisfied everyone with my performance and will continue to do so in the future.

Recently I was diagonosed with Hepatitis-B and for the treatment I need a lot amount of money. The amount can not be met by current financial condition. So I need financial assistance from the Bank's welfare fund and I hope that you will take necessary measures to grant me that. The required medicine and hospital documents are enclosed herewith.
